According to Sky TG24, the San Raffaele hospital in Milan switched two test tubes during an assisted reproduction procedure in July and implanted an embryo in the wrong patient. The error occurred because, according to the hospital itself, there was "an incorrect interpretation of the patient sequence in the operating program," and the embryo was transferred to the second patient on the list. The error was discovered minutes after the procedure was completed, when the responsible biologist consulted the program again and identified the mix-up.
The couple who received the wrong embryo were informed immediately and agreed to a procedure to stop the implantation. The other couple, who were the owners of the mistakenly transferred embryo, were only notified the following day, after the woman received the correct embryo.
